Industrial Accident Investigation & Analysis

We facilitate a scientific approach to accident investigation and analysis within a team of your personnel, who between them will have knowledge of work processes local to the incident site, your HSE Management system, your organisation and your culture.

• First we create an investigation plan based on initial reports and the experience of the team.
• Then we gather the evidence using the frame of ‘Papers’, ‘Positions’, ‘Parts’ & ‘People’, again using scientific methods and discipline experts as required.
• We then validate our investigation plan ensuring that every piece of evidence is corroborated by at least one other before being accepted as fact.
• Next we analyse the underlying causes.
• Finally in conjunction with the process owner of the accident site we make recommendations that will prevent the same incident from happening again.
