INVESTIGATIONS

We facilitate a scientific approach to accident investigation and analysis within a team of your personnel, who between them will have knowledge of work processes local to the incident site, your HSE Management system, your organisation and your culture.

TRAINING

We develop & facilitate training in industrial accident investigations and analyses. Our training replicates the scientific appoach used in the methodology. We focus first on creating multiple possible scenarios based on the limited facts available combined with the knowledge of the team with respect to what should have existed. The next step is to gather evidence to validate those scenarios and finally we conduct an underlying cause analysis to find the systemic causes that led to the incident

COACHING

We provide coaching to and quality assurance for organisations using Trilogy Facilitation  incident investigations and analyses processes.

MENTORING

We develop internal capability in client organisations to provide their own coaching & quality assurance.
